The College of St. Scholastica

All forms must completed and submitted electronically, EXCEPT the physical examination form and prescription medications form, which must be printed out, completed by the physician, physician assistant, or nurse practitioner completing the physical, and then returned to the athletic training department: All forms must be on file with the Athletic Training office for practice and competitions for every student-athlete at St. Scholastica.

*NOTE: "New" athlete is defined as any prospective student-athlete that did not practice with a St. Scholastica team during the 2013-14 academic year.
